Day Program Counselor

We are seeking a motivated individual who would provide direct care and help assist the participants with the day to day support at our day program. Some of the daily duties would be to assist with the implementation of individuals’ behavior plans, ensure completion of daily notes and tracking, and to provide direct support to the participants in the field.

We are looking for an individual who has experience working with individuals who have developmental disabilities and working with behaviors. One needs to be excellent with organizational and communication skills. Applicants are recommended to be QMAP and CPR
certified.

This is a position will start our part-time and lead into a full-time position. There are no benefits available with this position. We are a small, but growing organization and are therefore looking for an energetic, self-motivated individual.


Live-in Caregiver

We are looking for a compassionate, energetic person or couple who would like to live alongside and support adults with developmental disabilities. The caregiver lives in our house and is responsible for working weekends, weekday mornings and weekday nights, with weekdays generally free. The caregiver will also have some week nights and weekends off. This is a great opportunity for students, retirees, couples or someone with a work at home business!

Live-in responsibilities include daily data tracking, medication administration, house organization, teaching life skills and monitoring the health and safety of residents.

A successful candidate will be creative, organized, self-motivated, compassionate and patient. Experience with developmental disabilities is preferred but not necessary. The desire to be part of our community and the larger Loveland community is a must.

Live-in coworkers receive room and board and a monthly stipend.
 

Host Home Providers

Primary Duties: (Depending on level of client, care may include additional duties)

 

  • Supervise, support, and assist client as provided in their individual Service Plan
  • Document daily activities related to this team plan (diet, medication administration, social activities, chores, health / behavior issues, personal cash purchases, etc.)
  • Care is for 24/7.
  • Transport and attend at all medical, dental and other health care service appointments and provide required follow-up reports.
  • HHP provides emergency care and is responsible for BURS Back-up-Residential Service, scheduling and fees.  All BURS providers must be approved by CLS to be in compliance with all the training and certifications of the HHP.
  • Attend CLS HHP meetings.
  • Must attend regular meetings with client’s team from CLS and Foothills Gateway Inc. to review individual service plan.
  • Support family choice (visits to HHP home and/or client to family)
  • Purchase all food and provide adequate nutrition and meet any special dietary requirements.
  • Assure client's rights (as required by CLS and the service team) are up-held.
  • Client shall share in a family home setting and partake, with the support of the HHP, in the community activities of CLS (includes dinners, parties, festivals etc).
  • Participate in CLS community by attending and hosting community dinners, events and meetings
  • HHP and their home will be monitored on a regular basis by CLS for compliance to all required health and safety issues.
  • The HHP must notify CLS if they or anyone in the home is charged and/or arrested.

 

Education & Experience

  • Must have at least 1 year of Host Home or equivalent experience.

 

Requirements

  • HHP must own or be the primary renter of the dwelling in Loveland or surrounding area.
  • Client must have their own bedroom.
  • HHP must sign an annual contract with CLS and undergo recertification of all required training.
  • Undergo background checks for provider and all other adults residing in the home (some fees may be paid by the HHP)
  • Provide Reference checks
  • Provide DMV and auto safety checks, auto and home owner’s insurance compliance, CO HHP liability insurance
  • Completion of all necessary training (some fees will be paid by the HHP)

AmeriCorps Live-in Coworker

AmeriCorps members commit to a year of service to our organization as live-in coworkers supporting our participants in both our residential and day programs.  We currently have all of our positiions filled.
